Health Links

Late in 2012 the Ministry of Health and Long Term Care announced community Health Links with the intention of mobilizing the delivery of coordinated care services across the continuum of care for those with complex conditions. The evidence suggests that integrating services will increase quality and efficiency as well as access, safety and patient satisfaction. Health Links provide a unique and important opportunity for integration that can maximize patient accessibility to services and minimize duplication through streamlined transitions in care between all those who provide care for these patients.

There are currently 69 Health Links sites covering all LHINs, and Ontario’s physicians are integral to their success. Many new Links are coming on line -- check back soon for an updated list.

Health Links will initially target patients with complex conditions, who make up 5 percent of the population but consume 66 percent of health services. The goal is for the program to represent all Ontarians, with new patient groups phasing in regularly.

This site is designed to provide information, to hear your questions, and to eventually receive your feedback about Health Links. As this model of care evolves in our province, so will this website with new information being provided as it becomes available, and more opportunities for you to provide input and ask questions.
