FHO+: Continuity of care payment adjustment calculator

The calculator below is designed to help you understand whether your quarterly continuity of care measure will affect your future capitation payments.

Recall:

  1. Related measurement quarters are three quarters apart. On the calculator below, they are indicated by the coloured bands: blue, grey, and white.
  2. If both related measurement quarters are below 75 per cent, an adjustment will be applied to a future capitation payment
  3. The adjustment is applied two quarters after the second related measurement quarter
  4. The value of the adjustment is 15 per cent of the first related measurement quarter's total base capitation value

Calculator instructions

  1. Enter your average monthly base capitation payment into the box indicated.
    • This will auto-populate all capitation payment fields.
    • If you do not enter a number, a default payment of $15,000 per month will be used.

     OPTIONAL: You can enter different capitation payments for each month.

    • If you enter these manually, make sure to press ‘Enter’ after each entry.
    • Alternatively, you can use the up/down arrows next to each capitation payment box to change the payment

  1. Change the sample continuity of care rates in the third column, under ‘Your quarterly continuity of care rate (%)’.

  1. See whether the updated continuity of care rates you entered will result in a payment adjustment, by looking at the fourth column (‘Capitation adjustment’).
