FHO+: Continuity of care payment adjustment calculator
The calculator below is designed to help you understand whether your quarterly continuity of care measure will affect your future capitation payments.
Recall:
- Related measurement quarters are three quarters apart. On the calculator below, they are indicated by the coloured bands: blue, grey, and white.
- If both related measurement quarters are below 75 per cent, an adjustment will be applied to a future capitation payment
- The adjustment is applied two quarters after the second related measurement quarter
- The value of the adjustment is 15 per cent of the first related measurement quarter's total base capitation value

Calculator instructions
- Enter your average monthly base capitation payment into the box indicated.

- If you do not enter a number, a default payment of $15,000 per month will be used.
