FHO+: Group Management Leadership Payment

What is the GMLP?

The Group Leadership Management Payment (GMLP) is an administrative payment to the FHO group that compensates for leadership of the FHO. Ensuring compliance with the FHO contract is a key leadership responsibility, including required after-hours coverage and comprehensive care. Communicating with the ministry is another important component of FHO group leadership. Every FHO group must elect a lead and an associate lead to represent the group.

It is up to the discretion of each FHO group how the GMLP is allocated. Each FHO is different and may have different ways of allotting the payment among group leaders. We recommend that group governance agreements clearly outline how the GMLP will be allocated.

The addition of an Enhanced GMLP on April 1, 2026

Before April 1, 2026

The GMLP is $1 per rostered patient per fiscal year, prorated daily up to a maximum of $25K per year.

In addition to being available to FHOs, the GMLP is also available to FHNs and RNPGA physician groups.

After April 1, 2026

An Enhanced GMLP (E-GMLP) on top of the existing GMLP will pay $4 per rostered patient, up to a maximum of $100K.

This means that in total (for GMLP + E-GMLP), group lead physicians will get $5 per rostered patient, up to a maximum of $125K per year. 

The E-GMLP is only available to FHOs.

The current GMLP and the new Enhanced GMLP combined will pay each FHO a minimum of $25,000 per year.

Payment of the GMLP and E-GMLP

Payment for the existing GMLP will continue to be paid monthly. The payment will be to the FHO group.

Payment for the Enhanced GMLP (E-GMLP) is effective April 1, 2026. It will ultimately be paid monthly starting April 1, 2027; however, for the first year, the payments will be quarterly. FHOs don’t have to do anything to start receiving the E-GMLP. The payments will show up in the form of a deposit into the group account. 

FHO leadership responsibilities

It is up to the individual FHO how they want to divide responsibilities among the FHO lead, associate lead and other members of the FHO. While the group may determine how best to share these responsibilities, the FHO lead (and associate lead where applicable) is ultimately responsible for completion of forms and communication with the ministry.

These responsibilities include:

  • Scheduling appropriate after-hours availability and care
  • Collecting signed copies of the physician declaration form from all physicians in the group
  • Interviewing physicians interested in joining the FHO
  • Completing new FHO physician enrollment (must be signed by the FHO lead)
  • Completing new locum enrollment (must be signed by the FHO lead)
  • Mentoring new FHO physicians
  • Monitoring past and future outside use
  • Scheduling and chairing meetings of the FHO
  • Initiating and organizing reviews of the FHO governance agreement

The GMLP can be used to compensate members of the FHO for this work.

Note that if you use a group management company and they ask for a share of your GMLP, you can push back on this request. The GMLP is intended to be used for internal leadership of the FHO, not paid to third-party companies.

Communicating with the ministry

While the above activities can be performed by the FHO lead, associate lead and/or other members of the FHO, one responsibility that falls exclusively to the FHO lead is communicating with the ministry.

The FHO lead (not the associate lead) is the main point of contact between the FHO group and the ministry. The ministry will only send official documents to the FHO lead. They will also only accept submissions that originate from the FHO lead.

Related to communicating with the ministry, the FHO Lead must: 

Keep their contact information up to date with the ministry, especially their email

Act as the main point of contact between the FHO group and the ministry

Receive official documents from the ministry

Send official documents to the ministry on behalf of the group or individual members

Take appropriate action when documents are received from the ministry and share information with other members of the group

Sign all required forms, applications and contracts

Respond to ministry audits and provide any requested documentation on behalf of the group or individual members (e.g., financial records, enrolment‑related forms and declarations)

Maintain an organized and transparent system for storing and tracking all FHO documents and correspondence
